Registration Form Instructions

1. Each person attending the Poingdestre Family History Tours must register and make payments by the deadlines.
2. Down payment is required at the time of registration. 
3. Deadline to submit completed Registration Form with down payment must be postmarked on or before January 3, 2025.  Deadline to submit final payment must be postmarked on or before March 3, 2025.  You may pay in full on or before January 3.
4. Review all information on this tour web site before completing the Registration Form so you are familiar with your responsibilities.
5. Read the Terms and Conditions as they apply to everyone registering for the tours.
6. Print and complete the Registration Form  The Jersey Tour has Sold Out!

Payment Methods

1. Check:
   • Indicate on the Registration Form that you are paying by check.
   • Write the check to "PDA." 
   • Mail your check and completed Registration Form to the address listed on the form.
2. Credit or debit card:
   • Indicate on the Registration Form that you are paying via credit card.
   • Go to our Online Payment page and follow the instructions there.
   • Mail your completed Registration Form to the address listed on the form.
